当法人は、ITIN/EIN（米国連邦納税者番号）取得サポートサービス(以下、「本サービス」といいます)に関連してお客様からお預かりする個人情報を、次の通りお取り扱いさせていただきます。ご同意の上、個人情報の提供をお願い申し上げます。 ■個人情報取り扱い事業者の名称 アクタス税理士法人 1.個人情報の利用目的 当法人はお客様の個人情報を以下の目的で保有し利用いたします。 (1)お客様からの本サービスに関するお問い合わせに回答するため (2)お客様のご依頼に基づき、本サービスを提供するため (3)上記に付随する事務管理業務のため 2.個人情報の保護 当法人はお客様の個人情報を漏えい、滅失又は毀損から保護するために必要かつ適切な措置を講じます。 3.個人情報の委託 当法人は、業務の一部を上記利用目的の範囲内で委託先に委託する場合があります。その場合は、個人情報の適正な管理体制を備えている機関のみを選定し、適切な管理・監督をいたします。 4.個人情報の第三者提供 当法人は以下の場合を除き、本サービスに関連してお客様からお預かりした個人情報を第三者へ提供いたしません。 (1)あらかじめご本人の同意をいただいている場合 (2)本サービスの提供のために、弁護士、司法書士、行政書士等の専門家に提供する必要がある場合 (3)人の生命、身体又は財産の保護に必要な場合でご本人の同意を得ることが困難な場合 (4)司法機関、行政機関等から法令に基づき提供を求められた場合 (5)当法人が合併、分社化、営業譲渡等で、個人情報を提供する場合 　　 5.個人情報の開示等の請求 お客様は当法人に対して自己に関する個人情報の利用目的の通知、開示、内容の訂正、追加又は削除、利用の停止、消去及び第三者への提供の停止を請求することができます。ただし、以下の場合はご請求には応じられません。 (1)ご本人又は第三者の生命、身体、財産その他の権利利益を害する恐れがある場合 (2)開示することにより業務の適正な実施に著しい支障を及ぼすおそれがある場合 (3)法令に違反することとなる場合 6.任意性について 個人情報のご提供は任意ですが、ご提供いただけない場合は、お問い合わせに回答できないこと、本サービスを提供できないことがあります。 7.要配慮個人情報について 本サービスに関連して、パスポート、パスポート認証書類、運転免許証、米国申告書等、要配慮個人情報を含む書類原本またはその写し等をお預かりすることがあります。 8.お問合せ窓口 個人情報の開示等ご請求、個人情報保護管理者へのご連絡、その他ご不明な点に関するお問合せは、下記までお願いいたします。 個人情報保護管理者　粕谷　直人　　東京都港区赤坂3-2-6 赤坂中央ビル7F　Email info@actus.co.jp 個人情報お問合せ窓口　総務部責任者　　同上 Actus will handle personal information, including sensitive information, that we will receive from the customers in connection with the ITIN / EIN (Federal Tax Identification Number) application support service (hereinafter referred to as "the Service") as follows. Please read the following terms and condition carefully before you provide us with any personal information, and provide us with it only when you agree to the followings: ■Personal information handling company Actus Tax Corporation 1. Purposes of Use of Personal Information Actus shall hold and use the personal information of the customer for the following purposes. (1)To respond to the customer’s inquiries about the Service (2)To provide the Services requested by the customer (3)For the administrative works incidental to the above 2. Protection of Personal Information Actus shall take necessary and proper measures to protect the personal information of the customer from leakage, loss or destruction. 3. Entrustment of Personal Information Actus may entrust a part of its operation to an outside subcontractor within the scope of the purpose of use mentioned above. In such case, Actus shall select only a subcontractor with proper management system of personal information and properly manage and supervise such subcontractor. 　　 4. Provision of Personal Information to Third Parties Actus shall not disclose or provide the personal information of the customer to a third party except: (1)where the prior consent of the individual concerned has been received; (2)where necessary to provide the personal information to experts such as attorneys, judicial scriveners, administrative scriveners etc. in order to provide the Services; (3)where necessary in order to protect a life, body, or property of a person, and difficult to obtain the consent of the individual concerned; (4)where requested by a judicial organ or a government and administrative organization etc. based on laws; (5)where necessary to provide personal information due to a merger, corporate separation or business transfer etc. 5. Request to disclose, delete and amend the Personal Information The customer may request Actus to notify him or her about the purposes of use of his or her own personal information, to disclose such personal information, to make any necessary correction, deletion or addition in case of error or change therein, to suspend use or delete it in case of unjustifiable use, and to suspend the provision thereof to a third party. However, this paragraph does not apply to the following cases: (1)where disclosure is likely to harm a life, body, property, or other rights or interests of the individual concerned or a third party; (2)where disclosure is likely to seriously interfere with the proper business operation of Actus; (3)where disclosure would violate any law or regulation. 6. Voluntariness The provision of the personal information is voluntary. However, we may not be able to respond to an inquiry or provide the Services if you do not provide us with the information. 7. Sensitive information Actus may request to deposit originals or copies of the documents containing sensitive information, such as a passport, certified copy of passport, driver's license, US tax return form etc. in connection with the Service. 8.
